The challenge
Services for vulnerable children and families were being provided by different teams across different organisations and locations, resulting in a disjointed and uncoordinated experience for children and their families.
The solution
Development of ‘family hubs’, where integrated services are delivered to children and families by professionals working together through co-location, data-sharing and a common approach to their work. Families only have to tell their story once and service provision is integrated.
Impact
There is no ‘wrong front door’ for families, they receive the right service, at the right time, at the lowest possible level of service involvement, being able to self-help where possible. Families have a better understanding of available services and how to access them; staff have a better understanding of the roles and remits of other services and are actively seeking opportunities to co-deliver where this will lead to better outcomes for families.
